Axis Spine & Disc

How we text

Axis Spine & Disc (Back-Trac LLC) texts a person only after they check a box for it. Care texts and occasional offers are two separate, unchecked, optional boxes everywhere we ask. Neither is required for care.

What we send

  • Care texts (the first box): visit reminders and confirmations, check-ins, intake and payment links, receipts and replies to your messages. Some come from Axis, our automated assistant. Health details are never in a text; they go by secure link.
  • Occasional offers (the second box, optional): promotional texts, a few times a year at most.

Every consent is stored with the boxes checked, the version of the wording shown, where it was given and when. Then the clinic's number sends this confirmation (", including occasional offers" only when that box was checked):

Axis Spine & Disc: Thanks for subscribing to visit reminders, care updates and replies, including occasional offers. Reply HELP for help. Message frequency may vary. Msg&data rates may apply. Consent is not a condition of purchase. Reply STOP to opt out.

1. Paper New Patient form (front desk)

When: every new patient fills it in at the front desk when they check in for the first visit (and anyone whose mobile number changes). It has the mobile phone field, the two separate boxes, the disclaimer below and a signature and date line. The front desk records the checked boxes in our patient system the same day, which sends the confirmation text above. The signed form stays in the chart.

Disclaimer printed on the form:

By checking a box below, signing this form and providing your phone number, you agree to receive the SMS messages you checked from Axis Spine & Disc: visit reminders and confirmations, check-ins, intake and payment links, receipts and replies to your messages, and, only if you check the second box, occasional promotional offers. Message frequency may vary. Standard Message and Data Rates may apply. Reply STOP to opt out. Reply HELP for help. Consent is not a condition of purchase. Your mobile information will not be sold or shared with third parties for promotional or marketing purposes.

2. Online intake

Where: a secure, one-time link (portal.axisdisc.com/#/intake/<code>) the clinic sends after a visit is booked. Its texting step shows the same two unchecked, optional boxes, each with the full disclosure below; Continue works with neither checked. Checking a box sends the confirmation text above.

3. Website forms

Where: axisdisc.com/schedule/ and axisdisc.com/contact/ (the same form appears at the bottom of our condition and offer pages), and the Spine Score questionnaire. The phone number is required so we can call back about the request; two unchecked, optional boxes sit directly under it, and the form submits with neither checked. Privacy Policy and Terms are linked on the form. Checking a box sends the confirmation text above once the request reaches the clinic.
