These Terms govern your use of the Axis Spine & Disc website and the services of Back-Trac LLC, doing business as Axis Spine & Disc. By using the website or scheduling an appointment you agree to these Terms.

Not Medical Advice

Content on this website is for general information and does not constitute medical advice, diagnosis or treatment. Nothing on this site creates a doctor–patient relationship. If you have a medical emergency, call 911.

Evaluation & Candidacy

Every patient begins with an evaluation to determine whether our nonsurgical approach is appropriate. Not every patient is a candidate. Treatment recommendations are made only after examination and are provided to you in writing.

Pricing & Payment

The initial offer — evaluation + 3 treatment visits — is $149, payable at the first visit. If we determine you are not a candidate, you are refunded in full. Any care beyond the initial visits is recommended in person, in writing, and is priced and agreed to before it begins. Fees are self-pay; we can provide itemized receipts for health savings or flexible spending accounts.

Appointments & Cancellations

Please give at least 24 hours' notice to reschedule or cancel an appointment.

Website Use

You agree not to misuse the website, interfere with its operation, or submit false information through our forms. Website content, including text, images and the Axis Spine & Disc logo, is owned by Back-Trac LLC and may not be reproduced without permission.

Limitation of Liability

To the fullest extent permitted by law, Back-Trac LLC is not liable for indirect, incidental or consequential damages arising from use of the website. Nothing in these Terms limits liability that cannot be limited by law.

Governing Law

These Terms are governed by the laws of the State of Kansas.
