These Terms govern your use of the Axis Spine & Disc website and the services of Back-Trac LLC, doing business as Axis Spine & Disc. By using the website or scheduling an appointment you agree to these Terms.

Not Medical Advice

Content on this website is for general information and does not constitute medical advice, diagnosis or treatment. Nothing on this site creates a doctor–patient relationship. If you have a medical emergency, call 911.

Evaluation & Candidacy

Every patient begins with an evaluation to determine whether our nonsurgical approach is appropriate. Not every patient is a candidate. Treatment recommendations are made only after examination and are provided to you in writing.

Pricing & Payment

The first visit (consultation, imaging review, examination and a first treatment when appropriate) is $79 for new patients, with no obligation. New patients only. Any care beyond the first visit is recommended in person and agreed to before it begins.

Appointments & Cancellations

Please give at least 24 hours' notice to reschedule or cancel an appointment.

Text Messaging (SMS) Terms

Program: Axis Spine & Disc text messages. Care texts (if you agree to them): visit reminders and confirmations, check-ins, intake and payment links, receipts and replies to your messages, some sent by our automated assistant, Axis. Promotional texts (only if you separately agree to them): occasional offers, a few times a year at most.

How you agree: by checking the box for that kind of text on our website form (/schedule/ or /contact/), the only way to sign up for our texts. Consent is not a condition of purchase. Details and the exact wording: how we text.

Frequency and cost: Message frequency may vary. Message and data rates may apply. Reply STOP to opt out.

Opt out and help: Reply STOP to any text to cancel; you will receive one confirmation and no further messages. Reply START to opt back in. Reply HELP for help, or call (913) 355-5998.

Carriers are not liable for delayed or undelivered messages. Mobile opt-in information is not shared with third parties or affiliates.

Website Use

You agree not to misuse the website, interfere with its operation, or submit false information through our forms. Website content, including text, images and the Axis Spine & Disc logo, is owned by Back-Trac LLC and may not be reproduced without permission.

Limitation of Liability

To the fullest extent permitted by law, Back-Trac LLC is not liable for indirect, incidental or consequential damages arising from use of the website. Nothing in these Terms limits liability that cannot be limited by law.

Governing Law

These Terms are governed by the laws of the State of Kansas.
